As the festive season takes shape, alcohol often becomes part of the celebrations, family gatherings, office parties, weddings, and year-end holidays. For many, a drink is seen as harmless enjoyment. But this is also the time when hospitals see a surge in alcohol-related illnesses. From a medical standpoint, no amount of alcohol can be considered completely safe.
How alcohol affects the body varies widely from person to person. Genetics play an important role, as do sex, body composition, overall health, and how frequently and for how long someone drinks. Men and women process alcohol differently, and drinking patterns matter just as much as quantity.
Both regular drinking, even in small amounts, and occasional heavy drinking pose serious health risks. While chronic low-level consumption can lead to long-term damage such as liver disease and increased cancer risk, binge drinking even if infrequent can cause acute complications and contribute to cumulative harm over time.
Clinically, alcohol intake is measured in grammes of pure alcohol rather than by the size of a glass or bottle. This is why doctors refer to a “standard drink”. Different alcoholic beverages contain different concentrations of alcohol, but one standard drink contains about 14 grammes of pure alcohol. The liver can only break down roughly one standard drink per hour. During celebrations, when drinks are consumed faster than the liver can process them, excess alcohol remains in circulation and causes harm to multiple organs.
One of the most dangerous aspects of alcohol-related disease is how silently it develops. By the time symptoms appear, damage to the liver or other organs is often already advanced. Alcohol does not only affect the liver; it also damages the pancreas, brain, heart, nerves, and the digestive system.
Early symptoms may be subtle and easily overlooked, particularly when people dismiss them as stress or fatigue from the holiday rush. These include persistent tiredness, loss of appetite, nausea, and general weakness. As damage progresses, yellowing of the eyes and skin, known as jaundice, may develop. Fluid may accumulate in the abdomen, causing visible swelling. In more severe cases, patients may vomit blood or pass black stools due to internal bleeding.
Confusion and excessive drowsiness are especially serious warning signs. when the liver is damaged, it can no longer effectively clear toxins from the blood. These toxins gradually accumulate and affect the brain, leading to confusion, altered behaviour, and in advanced cases, loss of consciousness symptoms that are sometimes mistaken for simple intoxication.
Alcohol also has profound long-term effects on the brain. Continued use can lead to memory loss, reduced concentration, and cognitive decline. Some individuals develop alcohol use disorder, characterised by dependence and loss of control over drinking. When heavy drinkers suddenly stop consuming alcohol, they may experience alcohol withdrawal syndrome, which can include tremors, seizures, severe confusion, and can be life-threatening without medical supervision.
Alcohol increases risk of hypertension and arrhythmias (including atrial fibrillation). Chronic use can cause peripheral neuropathy (numbness, tingling). Alcohol irritates the stomach lining, leading to gastritis, ulcers, and GI bleeding. Strong caution against mixing alcohol with medications, a common occurrence during the holiday season when people self-medicate for pain, flu, or hangovers.
Many antibiotics should not be taken with alcohol, as the combination can trigger chemical reactions that cause flushing, severe vomiting, dizziness, and abdominal pain. Even commonly used painkillers are risky. Mixing alcohol with drugs such as aspirin greatly increases the risk of gastritis, ulcers, and internal bleeding.
Occasional binge drinking is not harmless. Forceful vomiting after heavy drinking can tear the digestive tract and result in life-threatening bleeding. In extreme cases, alcohol poisoning can occur. Because alcohol has a sedative effect, it can slow breathing, cause convulsions, and lead to loss of consciousness. These symptoms require urgent medical attention and are frequently seen in emergency departments during festive periods.
Inside the body, alcohol-related liver disease follows a predictable but dangerous progression. It often begins with fatty liver, which may cause no symptoms and is usually reversible if alcohol use stops early. Continued drinking can lead to alcoholic hepatitis, an inflammatory condition that can be severe and life-threatening. Over time, repeated injury results in liver cirrhosis permanent scarring that severely impairs liver function.
Despite this, the liver is one of the most forgiving organs in the body. When damage is identified early and alcohol consumption is reduced or stopped, the liver has a remarkable ability to regenerate and recover. Doctors rely on several tests to detect alcohol-related damage, even before symptoms develop.
Blood tests, commonly known as a liver panel, assess liver enzymes and can indicate injury, though they may miss early disease. Ultrasound can detect fatty liver and structural changes, while specialised tests such as Fibro Scan can identify early fibrotic changes before cirrhosis develops.
Alcohol is also a leading cause of chronic pancreatitis and a major contributor to several cancers, including cancers of the liver, throat, oesophagus, breast, and colon. The risk increases with both the amount of alcohol consumed and the length of time a person drinks over their lifetime.
Ultimately, alcohol-related illness is largely preventable. The danger lies in how quietly damage accumulates over time, often without obvious warning signs. As celebrations continue, people should be mindful of their drinking habits. Reducing or stopping alcohol early can prevent irreversible disease and save lives. During this festive season, moderation or choosing not to drink at all may be one of the most important health decisions a person can make.
